The application is to be completed via a government portal operated by the Directorate General of Immigration Ministry of Law and Human Rights to obtain an e-VOA - a single-entry visa that is valid for stays of up to 30 days with the purpose of tourism, government visits, business meetings, goods purchasing, or transit only. It is advised to save the QR code to a mobile device or print it out ready for arrival. If you arrive at Bali customs without a QR code, you will be asked to complete the form and be required to queue again. The official form itself can be found here.
